eCQM Data Integrity & EHR Interoperability
When a quality measure reads zero, check the build first. The documentation is usually there. It is not reaching the structured field the measure reads. Most zeros trace to a measure that was never switched on, a lab result filed in free text, a code set that was never mapped, or a vendor export that leaves patients out of the file. I trace the gap from the chart to the QRDA file, name the owner, and stay in the vendor session until the measure computes. Current contract: 100+ independent practices across 27 EHRs.

Workflow design, build validation, hands-on training at the workstation and support after go-live, from a nurse who has used these systems on the floor.
Adoption & Change Management
Staff learn the new workflow in their own EHR and show it back. Adoption is audited at 30, 60 and 90 days, and a named successor keeps it running.
SOPs & Workflow Documentation
Standard operating procedures, onboarding and handoff guides written so the work keeps going after the engagement ends.
